Supporting Local Solutions to Improve Health Across Louisiana

Community organizations across Louisiana are receiving support to expand programs that improve access to care and address the factors that influence health.

Six grants totaling $12,000 have been awarded to nonprofits, healthcare providers, and schools working on innovative, community-based health initiatives. These projects focus on addressing social drivers of health, increasing access to preventive care, and improving outcomes for families and individuals across the state.

“Funding innovative projects that address health-related social needs allows even small grants to make a big impact,” said Yolanda Wilson, Senior Vice President of Quality Improvement at Louisiana Healthcare Connections. “These grants invest in local organizations that are creating meaningful change in their communities.”

Each grant supports a unique, locally driven solution:

  • North Oaks Health System (Tangipahoa Parish)
    Establishing a food pantry to address food insecurity among patients while building toward long-term sustainability.
  • CareSouth (Baton Rouge, East Baton Rouge Parish)
    Expanding transportation services to help residents access healthy food and essential resources.
  • Free ALAS (Orleans Parish, New Orleans)
    Growing psychoeducational programs for youth, focused on trauma support, resilience, and healthy coping skills.
  • Zion Helping Hands (Northeast Louisiana)
    Enhancing food pantry infrastructure by increasing storage and refrigeration capacity to improve food access.
  • Boys and Girls Clubs of Acadiana (Crowley, Acadia Parish)
    Installing a touchless water fountain to promote hydration and healthy habits among youth.
  • Methodist Health System Foundation (St. Bernard Parish)
    Expanding school-based support services to address student mental health and basic needs.

Together, these initiatives reflect a broader effort to support community-led solutions that meet people where they are and respond to the unique needs of each region.
