Healthcare providers across Louisiana are being invited to explore new tools designed to improve adolescent health outcomes and support the transition from pediatric to adult care.
A virtual Lunch and Learn session on June 17 will introduce the Adolescent Well-Care and Youth Health Care Transition Toolkit. The webinar is free to attend and offers healthcare professionals practical resources to strengthen care coordination and support youth during critical stages of development.
Developed in collaboration with the Louisiana Department of Health and the Louisiana Chapter of the American Academy of Pediatrics, the toolkit provides guidance for providers working with adolescents as they move into adult healthcare systems.
The toolkit includes quality improvement worksheets, referral guides, and best practices to help ensure continuity of care and reduce gaps in services during this transition period.
“The Youth Health Transition Toolkit is a vital resource for ensuring adolescents stay engaged with healthcare services as they transition out of pediatric care,” said Dr. Patti Barovechio, Statewide Nurse Consultant with the Louisiana Department of Health. “By equipping providers with actionable tools, we are helping Louisiana’s youth achieve healthier futures.”
The session will feature Dr. Barovechio, who brings decades of nursing and public health experience focused on strengthening care systems for children and adolescents. Her work has contributed to statewide efforts to improve youth transition resources and access to care.
“These peer-to-peer sessions focus on real-world healthcare challenges and help providers address the issues they see every day in their practices,” said Dr. Stewart Gordon, Chief Medical Officer of Louisiana Healthcare Connections. “By sharing practical solutions, we support providers in delivering high-quality care.”
The webinar may qualify for continuing education credits for physicians, nurses, and social workers, depending on accreditation.
