Supporting Community Health Initiatives Through Grant Funding

Community organizations across Louisiana are being invited to apply for funding to support programs that improve health and address barriers to care.

Up to $200,000 is available through the 2024 Community Health Grants cycle, supporting nonprofits, healthcare providers, and schools working on health-related initiatives across the state. Priority is given to projects that address social drivers of health, increase access to preventive care, promote health equity, and improve overall health outcomes.

Two funding opportunities are available. Major investment grants include three awards of $50,000 each, while an additional $50,000 will support smaller, targeted grants of $2,000 or less awarded throughout the year.

“Local organizations are uniquely positioned to address the needs of their communities,” said Jamie Schlottman, Plan President and CEO of Louisiana Healthcare Connections. “These grants help strengthen programs that create meaningful, lasting impact across Louisiana.”

Applications for major investment grants are due by August 2, 2024, with awards announced in September. Smaller grant cycles will open later in the year and accept applications on a rolling basis.
