Expanding Community Health Grants to Support Local Organizations Across Louisiana

Organizations across Louisiana are being invited to apply for funding to support programs that improve access to care and address the factors that influence health.

A total of $200,000 in Community Health Grant funding is available to nonprofits, healthcare providers, and schools working to advance health outcomes across the state. The funding supports initiatives focused on social drivers of health, preventive care, and health equity. Priority will be given to projects that expand access to care, address health disparities, and improve outcomes for communities with the greatest need.

“As we continue to advance health equity, we are committed to supporting organizations that are making a meaningful impact in their communities,” said Jamie Schlottman, CEO and Plan President of Louisiana Healthcare Connections. “By investing in these initiatives, we are working to build a healthier future for families across Louisiana.”

Two types of funding opportunities are available through this grant cycle:

  • Major investments
Three grants of $50,000 will be awarded to support larger-scale, high-impact initiatives.
  • Targeted community investments
An additional $50,000 will fund smaller projects of $2,000 or less, awarded quarterly to address specific community needs.

Eligible organizations are encouraged to submit applications for funding requests up to $50,000. Applications for major investment grants must be submitted by August 2, 2024, with award announcements expected in September.

Additional quarterly grant cycles are expected to open later in the year, with applications accepted on a rolling basis.

By supporting local organizations, this initiative helps strengthen community-based solutions that improve health, expand access to care, and address the unique needs of Louisiana communities.
